Faculty Personnel: Procedures and Forms

This page contains links to procedures, sample documents, and forms for the following consultation/reviews: performance consultation of probationary faculty; performance review and decision of probationary faculty; mandatory tenure decision; promotion to professor; annual face-to-face reviews of faculty; and merit procedures for faculty.  It is designed for department chairs, deans, constituency committee members, and other administrative personnel who are involved in:  the annual review of tenure-track faculty; tenure-track faculty who are applying for promotion and tenure; faculty who are applying for promotion to Professor; and departmental Merit Committee members.   Faculty who are applying for promotion and tenure or promotion to Professor should consult the UW-Whitewater Purple Book web site for information regarding policies and procedures.
