Consultations, Annual Reviews, Merit, & Post-Tenure Review

This page contains information for faculty and instructional academic staff on procedures and forms for the following: consultations and reviews of probationary (tenure-track) faculty; post-tenure review; annual face-to-face reviews of faculty and instructional academic staff; and merit information for faculty and instructional academic staff. Faculty or instructional academic staff who wish to review the forms used by departments in the consultation and review process will find sample documents and forms on the Chairs, Deans and Administrators page.

The information within each Tab (Year 1-Year 6), including the 2012 Routing Form, 2012 Performance Evaluation Form, 2012 Narrative, course evaluation summaries, peer reviews of teaching, summary of consultation or record of decision [department], Recommended Actions [Constituency Dean, Constituency Standards Committee, if applicable, and Provost & Vice Chancellor for Academic Affairs], and any rebuttals or Reconsideration Reports, should remain in the Purple Book for review in subsequent consultations/performance reviews. Information in the Appendices should be removed every year and retained by the faculty member until the completion of the review process for tenure/promotion to Associate Professor.

Consultation and Reappointment Reviews of Probationary (Tenure-Track) Faculty

Post-Tenure Review

UW System policies require that tenured faculty members undergo post-tenure review on a scheduled basis. The Provost's office forwards a list of faculty who are to undergo post-tenure review to deans at the beginning of the fall term. Post-tenure reviews are to be completed and reports submitted to the deans before the end of the fall term.

Annual Face-to-Face Review: Faculty & Instructional Academic Staff

An annual face-to-face review of faculty and instructional academic staff with "supervisors" is required. Goals are established during the face-to-face meeting in the fall term of odd-numbered years. The face-to-face meeting that occurs during the fall term of even-numbered years serves as an "interim check." During the subsequent fall term (the next odd-numbered year), the accomplishments of the goals for the previous two years are evaluated and a new set of goals is established for the next two-year cycle.

For probationary faculty, the face-to-face review should be combined with the Consultation or Reappointment Review. Face-to-face reviews of tenured faculty and instructional academic staff should follow departmental and college procedures.

Expedited reviews of Instructional Academic Staff

Instructional Academic Staff with less than 50% appointments may undergo an expedited review. Details on what is to be included in an Expedited Review are available at the Academic Staff Review Matrix.
