Administrative Affairs - Human Resources and Diversity

Faculty / Staff / LTEs (new hires and retirements/terminations)


The faculty/staff/LTE process incorporates the use of two new forms the UW-Whitewater Information Systems Access Request and Authorization Form and the UW-Whitewater Information Systems Compliance Form. Effective January 1, 2003, these forms will be used to grant employee access to the University's information systems and can be found in downloadable forms at http://www.uww.edu/Adminaff/hr/dfhr.htm (under the "Hiring Forms" area, click on " Access and Compliance Forms")

The UW-Whitewater Information Systems Access Request and Authorization Form is to be completed by the department supervisor for all newly hired classified, unclassified and LTE employees and submitted to the Campus Computer Helpdesk as follows:

Classified - Submit to the Helpdesk when submitting the Written Hire Reason for Classified and Project Appointment Form to Human Resources & Diversity (HR&D).
Unclassified - Submit to the Helpdesk when submitting the Hire Form to HR&D.
LTE - Submit to the Helpdesk when you notify HR&D of the LTE selected.


The UW-Whitewater Information Systems Compliance Form will be distributed to the new hire by HR&D as follows:

Classified - with the appointment letter requesting that the Compliance Form is signed and returned to HR&D.
Unclassified - with the contract requesting that the Compliance Form is signed and returned to HR&D.
LTE - HR&D will request the LTE to sign the Compliance Form at the time of LTE Orientation


Upon receipt of both the supervisor's signed/completed Access Request and Authorization Form and the employee's signed/completed Compliance Form, access will be approved and granted (or denied) and the supervisor will be notified. Please allow up to 2 weeks (most requests will be done sooner) for the access process to be completed. During this time, it is recommended that employees register for training. Subsequent requests by the supervisor to change the employee's security and/or access (in his/her current employment position) should be made via the Access and Authorization Request Form and are binding upon the initial Compliance form on file. Do not request access that has already been granted unless that access is to be modified.
