Services Provided: Residential care, intermediate care and skilled nursing care (84 beds) for mean and women of all races, creeds, and color and of all financial levels who are 65 years of age or older (in the skilled nursing section people 18 years of age and older can be admitted.) All apartments (100) are licensed as CBRF. We also have 40 Assisted Living units and a 24 unit Memory Care Residents. Fairhaven also prepares Meals on Wheels and participates in other community programs.

Area Served: Predominantly the state of Wisconsin; however, people have been admitted from all parts of the United States.
Source of Funds: Private payment for service, contributions for the United Church of Christ (individuals and congregations), private insurance companies, Title XIX & Medicare.
