Services Provided: Central Wisconsin Center (CWC) is one of three state-operated Intermediate Care Facilities for Mentally Retarded (ICF-MR). CWC provides residential/treatment services to children and adults with developmental disabilities. The residential facility currently serves 340 individuals with cognitive functioning levels of severe to profound mental retardation. The majority of the individuals served at CWC have multiple medical/personal care needs. A large number served have Cerebral Palsy, epilepsy, vision and/or hearing deficits, muscle skeletal problems and are totally dependent upon others for all personal cares. The care and treatment of individuals living at CWC is provided by an interdisciplinary team which typically includes an MD, RN, Resident Care Technician, Social Worker, PT, OT, Dietitian, Qualified Mental Retardation Specialist, Psychologist, Communication Specialist and other specialists as needed. In addition to the residential program CWC operates three short-term care programs which provide intensive assessment and evaluations to individuals with developmental disabilities living in the community. The short term care programs are: the Short Term Care Unit (STCU) for semi-ambulant to non-ambulant medically involved; the Developmental Evaluation Clinic (DEC) for ambulant individuals with mild behavior and/or communication challenges and the Short Term Assessment Program (STAP) for individuals with severe behavioral challenges.
Summary of Expected Responsibilities and Learning Outcomes of Student Interns: Under supervision, the intern will provide casework and counseling services to the individuals and the families/caregivers served in the Short Term Care Unit (STCU). These services will include preparation and coordination of short-term admissions and discharges, participation in team meetings, phone contacts and on-going liaison work with the families/caregivers and other responsible parties (i.e., responsible county DD Board, Human Service agencies, health care providers, public school, day program and respite care staff), preparation of care guides and discharge summaries.
The intern will also serve as an Interdisciplinary team member and provide information to the team as related to the family/guardian/primary caregiver needs and will act as an advocate for the individual being served with the involved community agencies.
