Summary of Expected Responsibilities and Learning Outcomes of Student Interns: Students would be based primarily at Hillside Manor Nursing Home with opportunity for additional limited experience in acute care, home care/hospice, Remembrance Home (dementia-specific CBRF/ and Memory Clinic. Field placement would prepare student to function as nursing home social worker. Learning outcomes include:

• Completing social history and MDS assessment
• Facilitating multidisciplinary care conferences
• Discussing/completing Advance Directives for Health Care
• Working with families of residents/facilitating family conferences
• Assessing resident's cognitive/emotional status
• Developing/implementing resident care plans to meet psychosocial needs
• Discharge planning
