Services Provided: LindenGrove is a 135-bed facility that provides skilled nursing care, as well as other compassionate services, to the segment population of geriatrics. We have a 17-bed sub-acute rehabilitation unit primarily for short-term patients with goals to return back to the community. There are Physical, Occupation, and Speech Therapy services provided on site. The social workers at LindenGrove are advocates for the residents as well as their families. We provide 24-hour skilled nursing care and Social Services is involved for the resident and their family.
Summary of Expected Responsibilities and Learning Outcomes of Student Interns: As a student intern, we at LindenGrove would like to see you have a certain number of residents for a caseload during the internship process. Both Social Service staff and the comfort level of the intern would determine the caseload. We would expect you to learn the roles of a long-term care social worker. You should get a good feeling of how a nursing home is organized and it's operations. You would also need to learn about the "Resident Bill of Rights" and being an advocate for the resident. Assessments such as geriatric mini-mentals, depression scales, social histories, and care planning would be part of the caseload requirements. Discharge planning and community services would be reviewed. LindenGrove-Waukesha wants the student intern to learn as much as possible about the social worker role and job expectations in long-term care. The student intern should see the social worker as an advocate for the resident and one who assists in helping them live a dignified life.
