Services Provided: Provide services to the senior population which focus on choice, independence and community involvement. Handles intake assessments and case manages an individual caseload. Designs individual care plans that promote community living and a quality lifestyle. Serve as a liaison to the public regarding department programs and community resources. Provide information, referral, advocacy, short-term assistance, and crisis intervention to older people, their families and caregivers. Arranges, refers, coordinates and monitors services provided through a wide variety of community resources. This agency is also responsible for intake services of Elderly Abuse reports.

Summary of Expected Responsibilities and Learning Outcomes of Student Interns: Interns will learn to be part of an interdisciplinary team and assume a role of entry-level Social Worker. Responsibilities include: Conducting and writing intake assessments via telephone calls or home visits. Perform information and assistance duties. Designing individual care plans that promote independence in a community setting. Learn to use computer to enter data and maintain current progress notes. Learn community resources and build relationships with these resources to promote ability to advocate for client. Attend staff meetings and express concerns, ideas and questions regarding individual case or caseload. Advocate for seniors.
