Submit the following materials to:

  1. Graduate School Application. An application may be requested from the grad school by email (gradschl@uww.edu), telephone (262-472-1006), or you may use the on-line application
  2. Graduate School Application Fee of $56.
  3. Official transcripts mailed from all schools attended. Currently enrolled students should include transcripts showing fall grades.
  4. Graduate Record Examination taken within the last five years
    (school code: 1921; dept. code: 0627)
  5. Applicants interested in a graduate assistantship should download and submit a graduate assistantship application.

International students should also see the Center for Global Education web site for additional requirements.

  1. Communication Sciences & Disorders program application
  2. One-page, double-spaced essay on why you would be a successful graduate student.
  3. Letters of recommendation secured and forwarded by 2/1/13 (minimum of two letters).

Note: If you DO NOT have a undergraduate degree in Communication Sciences & Disorders or equivalent, you will have to apply as a nondegree/special student. Please contact Dr. Bradley at bradleys@uww.edu for further information.

All applicants are responsible for ensuring a complete application. You will not be notified of missing documents. All graduate students begin the program in the Fall semester with the expectation of a full time credit load of 12 credits. Students with exceptional needs are accommodated on an individualized basis.
