Teacher Internship Application Process

Special Education students must contact the Special Education Field Coordinator of Directed Teaching or Department Chair for approval before applying to the Teacher Internship Program.
All students must pass the appropriate Praxis II test for their major before they will be approved as an intern candidate. However, we encourage students to apply to the Teacher Internship Program even if they haven't passed the Praxis II test yet.
Forms/documents necessary to apply for a teacher internship are listed below . Complete the forms online and print them, or print them out blank and fill in by hand (DO NOT STAPLE). These forms cannot be saved online. Adobe Reader is required. You can download a free copy at the Adobe Reader Homepage. Also, please contact Dr. Mason or Cheryl Wegner if you have problems with the forms.

THREE (3) Teacher Internship Reference Forms - Applicants must complete one F.E.R.P.A form to give/send to each reference. Applicants can give/send the Reference form to the reference or have the reference go online to complete it. If the reference completes it online, s/he can print the form, sign, and return to the Teacher Internship Office or to the applicant. Applicants must also complete one F.E.R.P.A. form for the Teacher Internship Office (See #20 below for assistance in completing the F.E.R.P.A form after clicking on "here" at the bottom of this page.)

FERPA - Click on COEPS FERPA Authorization form. (F.E.R.P.A. stands for Federal Educational Rights & Privacy Act.) For assistance in completing the F.E.R.P.A. form, see #20 below after clicking on "here"). Applicants should complete and sign one FERPA form for each of their references and give it to them to keep. The purpose of the F.E.R.P.A form is to give the reference permission to complete the reference form for you. Applicants must also complete one F.E.R.P.A. form for the Teacher Internship Office. This gives us permission to send your file to schools in need of interns. (See #20 below for assistance in completing the F.E.R.P.A form after clicking on "here" at the bottom of this page.)
