Application Procedure


Filling out the Application

  1. Print off the appropriate Professional Education application form.
  2. Fill out application by printing clearly (black ink is preferable)
  3. Indicate if you have transfer credits
  4. Indicate if you hold a previous bachelor's degree
  5. If you have declared a licensure minor, or plan to declare a licensure minor, fill in the name of minor(s). (This information is needed for possible determination of the Praxis II and/or Content Tests)
  6. Clearly print your name, student ID number, and phone number
  7. Read and sign the Memo of Understanding
  8. Attach appropriate material listed below
  9. Submit the completed application to the College of Education and Professional Studies Information Desk (Winther Hall lobby area) by the deadline listed on the application. Late applications will be returned to you. (Applications will not be considered as exceptions during Continuous Admission.)
  10. Results will be available at the College of Education and Professional Studies Information Desk (Winther Hall lobby area) on the Results Date lists on your application, unless otherwise notified by campus email. If applying during Continuous Admission, you will be notified by campus email when your results are available. You will need a picture ID to obtain your results.
  11. If admitted to Professional Education, you will be provided with a copy of your application which indicates the Praxis II and/or Content Exam(s) that will need to be passed before student teaching, interning, or practicum placements are made
  12. The College of Education and Professional Studies does not take responsibility for students who may take incorrect tests before official admission acceptance notification.
  13. Application determinations will be based first on those students who meet all basic eligibility requirements AND specified guaranteed GPA; Second consideration will be granted to applicants who meet the basic eligibility requirements; and third consideration will be granted to applicants who are considered an exception
  14. SUMMER ADMISSION only: If you would like your summer admission results to be mailed to you, you must attach a self-addressed, stamped envelope with your application materials.
  15. SUMMER ADMISSION only: Admission GPA will be the combined cumulative GPA using grades earned in Spring Semester courses.

Required attachments for students seeking their first bachelor's degree

  • UW-Whitewater unofficial transcript showing completion of (or concurrent enrollment in courses) of Phase 1 and Phase 2 orientation meetings, Wisconsin passing scores on the PPST (Praxis I) and/or Praxis CORE, ACT, SAT, or GRE, minimum of 40 completed credits, Communication 110, Foundations Block courses, and the Phase 2 portfolio or EDFOUND 214 Portfolio course (if applicable)
  • Academic Advisement Report (AAR) showing combined cumulative GPA (1st page of PDF)
  • If Communication 110 or any of the Foundations Block courses have been waived or substituted, official waiver/substitution forms must be on file with the College of Education and Professional Studies Information Desk (Winther Hall lobby area) and a full AAR (as a PDF) demonstrating waivers/substitutions must be submitted with application materials.

Required attachments for students who hold a bachelor's degree

  • Unoffical transcripts from ALL previous institutions (to verify combined cumulative GPA and number of credits)
  • Current UW-Whitewater unofficial transcript showing completion (or concurrent enrollment in courses) of the Phase 1 and Phase 2 meetings, Wisconsin passing scores on the PPST (Praxis I) and/or Praxis CORE, ACT, SAT, or GRE, Foundations Block Courses, and the Phase 2 Portfolio of EDFOUND 214 Portfolio course (if applicable)
  • PI-34 and Admission Checklist (filled out by appropriate Licensure Program Coordinator)
  • If Communication 110 or any of the Foundations Block courses have been waived or substituted, then the PI-34 and Admission Checklist must reflect corresponding waivers/substitutions
  • Praxis II and/or content exam score report or printed copy of Test Results page in WINS (if applicable)
  • Academic Forgiveness worksheet (if applicable)

Returning Ineligible Applications

Submit the completed application to the College of Education and Professional Information Desk (Winther Hall lobby area) by the deadline listed on the application. All required attachments must be included at the time of submission. If anything required attachments are missing, your application will be returned to you. However, you may re-submit your application once you have all required attachments as long as materials are submitted prior to the application deadline. Late applications will be returned to applicant. 

Continuous Admission is granted on a first received-first considered basis until all slots are filed in a particular program. Applications submitted in-person to the College of Education and Professional Studies Information Desk (Winther Hall lobby area) will be considered before those received by fax, mail, or through the drop-off box at Winther 2031. Students cannot apply as an exception during Continuous Admission. Summer Admission Only: Applications are accepted during the time period listed on the application. All summer applications will be held in our office until spring grades have been posted. Your admission will be determined using whatever spring semester grades are listed on your unofficial transcript at the closing of the summer admission period. Watch your unofficial transcript closely. Incomplete grades or grades not turned in by the instructor may or may not result in a lower GPA.

Notification of Admission Results

During Regular Admission cycles, results will be available at the College of Education and Professional Studies Information Desk (Winther Hall lobby area) on the pre-determined date listed on your application. You will be then be required to pick up your results at the Winther Hall Information Desk. A picture ID is required to pick up your results. Summer Admission Only:  Many students are not on campus at the time of results. For summer only, you may submit a self-addressed envelope and results will be mailed to you.
