How to Access Services

1. Be admitted to UW-Whitewater
2. Complete a Center for Students with Disabilities Application for Accommodations Form
3. Complete an Application for Project ASSIST
4. Submit appropriate documentation of your disability
5. Submit a personal statement
6. Submit two letter of recommendation forms from teachers

Students must be eligible for services through CSD in order to use Project ASSIST. Information and documentation will be reviewed to determine eligibility for Project ASSIST services.
