In the event of an emergency or if you feel your personal safety is threatened, do not use these forms. Please contact the UW-Whitewater Police Services by dialing 911.

CARE Team Reporting Form
Members of the UW-Whitewater community may use this form to report concerning behavior directly to the CARE Team.  The Reporting form is securely transmitted to the CARE Team Case Manager in the Dean of Students Office. The Case Manager reviews all concerns submitted and shares them with the CARE Team members.  CARE Team members review the reports and determine an appropriate course of action for each situation using team protocols, professional knowledge and past precedent. 

The Case Manager may contact individuals who have submitted a concern to obtain more detailed information. All concerns will be investigated and action will be taken to address them in a manner that provides a high level of care for each individual and protects our campus community.  Due to FERPA privacy constraints, members of the CARE Team will not be able to share information about the team's response with individuals who report concerns.

Sexual Misconduct Reporting Form
Members of the UW-Whitewater community must use this form to report any incident of sexual misconduct disclosed to them.  The reporting form is securely transmitted to the Dean of Students Office and the Title IX Coordinator. This form can be used to report: sexual assault, stalking, dating violence and domestic violence.  Once received, the Dean of Students Office, in collaboration with the Title IX Coordinator, will follow-up as required by federal law, including providing resources to the involved parties. 

The Dean of Students Office may contact individuals who have submitted a report to obtain more detailed information. Hate and Bias Reporting Form
Members of the UW-Whitewater community may use this form to report concerning behavior related to hate or bias incidents.  The reporting form is securely transmitted to the Dean of Students Office and the Hate/Bia Response Team.  The team will review all reports and determine appropriate follow-up to ensure we encourage and maintain a welcoming and equitable campus community.  Completing this form does not mean you are filing a formal complaint.  In many cases, incidents of bias do not violate campus policy, but instead offer opportunities for educational follow-up to improve our campus climate.

Crime Reporting Form
Members of the UW-Whitewater community may use this form to report crimes that took place on campus.  If you have already reported the crime to the University of Wisconsin-Whitewater Police, you do not need to complete this form.  All submitted forms will be routed to the UW-Whitewater Police Department.

Hazing Incident Reporting Form
Members of the UW-Whitewater community who have been victim of, or witness to, a hazing incident, are encouraged to complete this form.  This includes members of a student organization, Greek organization, intermural, sports team, or other organized student group.  Information provided will be transmitted to the Dean of Students Office for follow-up and investigation.
