Online Entry Forms

All events require a completed participation form. All entry forms need to be completed online at the links provided below. Paper forms will ONLY be available by request at the front desk of Career & Leadership Development. Online forms are due Wednesday, October 3 by 11:55pm. Paper forms must be completed and turned in to Career & Leadership Development, UC 146, by 4pm on Wednesday, October 3.

Directions for all forms (except Court and Senior Awards):

To sign your group up for participation in Homecoming events:

1. Go to https://my.uww.edu
2. Log in with your personal net-id and password
3. The following categories will have access to the sign-up pages: Classified/LTE/Grad Students, All Students and Student Employees
4. Sign up for those events your group will participate in
5. If you sign up for any component of the Homecoming Games and/or the Window Plexi Painting, make sure to submit your group's fees by the end of the Homecoming Info Meeting at 9:30pm on Monday, Oct 8th (UC Summers Auditorium)
