Intramural Entries
How do I enter?
You can download a NEW official entry form(FRONT)(BACK) or pick one up at the Office of Recreation Sports and Facilities (Room 100, Williams Center). Entries must be completed to enter all Intramural Sports activities. Incomplete and illegible entries will not be accepted.
The team captain must completely fill out the requested information at the top of the entry including team name, sport, division (Men’s, Women’s, CoRec), league (1st and 2nd choice), and captain and co-captain’s phone number and e-mail address. The section of the entry certifying that all participants are eligible must be signed by the captain.
A team entry must contain the names, phone numbers, and UW-W ID numbers of the captain and co-captain, and the names, UW-W ID numbers, and email addresses of at least two (2) additional team members, so there are a total of four (4) players. (Exception: Sports needing only one, two, or three persons may have less than the four required names and information.)
All entries must be turned into the Office of Recreation Sports prior to the registration deadline. All entries received after the deadline will be put on a waiting list. Entries can be submitted anytime the Office of Recreation Sports is open. The appropriate entry fee (cash or check only) must also be submitted with the completed entry form.
What if I have an additional player?
If you have a replacement or addition to your team you must complete an Addition Form. This MUST be present with you at your next game you intend to use the player at.
