Intramural Entries

How do I enter?

You can download an official entry form: FRONT SIDE | BACK SIDE or pick one up at the Office of Recreation Sports and Facilities (Room 100, Williams Center). The forms are BLUE. Entries must be completed to enter all Intramural Sports activities. Incomplete and illegible entries will not be accepted. MAKE SURE TO FILL OUT BOTH SIDES OF THE INTRAMURAL ENTRY FORM!

The team captain must completely fill out the requested information at the top of the entry including team name, activity, captain, league (M, W, CR), first choice for a division (i.e. 1A, 1B, 2A), and local address, phone and UWW E-mail of the captain. The team captain must also sign the section of the form certifying that all participants are eligible and that they have read and signed the waiver on the back.

A team entry must contain the names, UWW ID #, gender, phone numbers, and rank (grade) of at least four (4) team members. The team captain should fill out line #1 of the Entry Form. (Exception: Sports needing only one or two persons may have less than the four required names and information.)

All entries must be turned into the Office of Recreation Sports prior to the registration deadline. All entries are due by 4:00pm on the date listed (unless otherwise noted). All entries received after the deadline will be put on a waiting list and may be added to the league if there are openings. Entries can be submitted anytime the Office of Recreation Sports is open. The appropriate entry fee (cash or check only) must also be submitted with the completed entry form.

The captain must attend the Captain's Meeting as well. All Captain's Meetings are mandatory!

What if I have an additional player?

Teams are able to add players throughout league play (no roster additions are allowed during League Championships or playoffs). If you would like to add a player to your team, then you must complete an Addition Form. Please bring this form to your next contest (do not bring to the Recreation Sports Office). This MUST be present with you at your next contest.
