Dear Families;

Welcome to the UW - Whitewater Children's Center. In order for your child to be able to attend our center, we need you to fill out all the necessary forms. These forms are available either in our office or electronically by clicking here.

All forms need to be filled out and returned to our office before your childs first day of school. The Child Healthcare Report needs to be signed by a pediatrician and turned into our office within 2 weeks after your child starts school.
