Information for Individual Award

The individual award was created to recognize an individual making significant contributions to address environmental and sustainability issues.

To be considered for the award, the following information must be provided:

1. Nominee name, address, telephone number and email address (if available).
2. Nominator name, address, telephone number and email address (if available).
3. A description, which need be no longer than one page, briefly summarizing how the nominee has contributed to the achievement of environmental sustainability.
