Property Loss and Liability Incidents
Reporting Property Loss Incidents
The Office of Compliance and Risk Management (OCRM) coordinates property loss incidents and claims on behalf of UW-Whitewater. The current property loss deductibles are $1,000 for damaged or destroyed items, $1,000 for stolen property involving forced entry, and $2,500 for stolen property where there was no evidence of forced entry.
To report an incident and request an insurance reimbursement for a property loss, the following steps must be completed:
- Complete the General Incident Report Form (DOA-6441).
- Complete a Lightning Losses Affidavit, if applicable.
- Provide a police report for stolen or vandalized items.
- Submit a replacement quote or a repair estimate for the damaged property. Two (2) quotes or estimates are required for any property loss estimated to meet or exceed $2,500.
Once all required documentation is complete, submit the materials to ocrm@uww.edu. OCRM will review the claim and work with the UW System Office of Risk Management and the State of Wisconsin Bureau of State Risk Management to process the reimbursement.
Reporting Liability Incidents
The Office of Compliance and Risk Management (OCRM) coordinates the reporting and management of liability incidents involving students, visitors, guests, and other non-employees on behalf of UW-Whitewater. Employee work-related injuries or illnesses should be reported through the University's Workers' Compensation process coordinated by Human Resources.
Prompt reporting of liability incidents is essential to ensure the University can investigate the circumstances, preserve relevant information, coordinate with appropriate internal and external stakeholders, and attempt to resolve claims within applicable legal and UW System timeframes.
To report an incident that may result in a liability claim against the University or its employees, the following information should be submitted, as applicable:
- Complete the General Incident Report Form (DOA-6441).
- Provide photographs of the incident scene, property damage, or injuries, if available.
- Provide any supporting documentation related to the incident (e.g., witness statements, police reports, correspondence, contracts, maintenance records, or other relevant materials).
- Provide the names and contact information of any individuals involved in or who witnessed the incident.
- Preserve any physical evidence related to the incident, when appropriate, and notify OCRM if immediate assistance is needed.
Once all available documentation has been gathered, submit the materials to ocrm@uww.edu as soon as possible. OCRM will review the incident, coordinate any necessary follow-up with campus departments and external parties, and work with the UW System Office of Risk Management and the State of Wisconsin Bureau of State Risk Management to process the claim.
