Imputed Income and Health Insurance Benefits -Information and Calculator
Open enrollments for dental, vision and UW Employees Life insurance will be from Oct 5-Nov 13, 2009. Domestic partners can be added to dental and vision during this enrollment period. Once enrolled in dental and vision, subscribers must remain covered for the entire calendar year. Employee Reimbursement Account (ERA) enrollment dates are Oct 5-Nov 13, 2009. You must re-enroll annually. New for 2010 is the myFMBC Card. Participants in the Medical Expense Account will receive an electronic payment card to use when paying for prescription co-pays, over-the-counter items and other eligible expenses. This is voluntary. If you choose not to use the card, you will continue to submit a reimbursement form for eligible expenses. To access more information, please see this document.
Effective Jan 1, 2010, dependents are eligible for coverage until the end of the month in which they turn 27. There will be a tax consequence if the dependent is not eligible to be claimed as a dependent for tax purposes per IRS Code Section 152. The fair market value of insurance coverage provided to the non-tax dependent will be added to your earnings as imputed income. To review the imputed income calculator, please visit: http://etf.wi.gov/employers/benefit_programs_imputed_income.htm If you cover a non-tax dependent on any benefit other than health insurance, such as EPIC, VSP, or Anthem DentalBlue, the monthly premium will be taken on a post-tax basis.

15th Annual Outstanding Women of Color Award
We are accepting nominations for the Outstanding Women of Color Award from UW-Whitewater. If you would like to nominate an individual for the "Women of Color" Award who has made outstanding contributions to diversity and women's studies at UW-W, please submit a one-page summary of the honoree's contributions by Friday, December 8, 2009. Please see selection criteria under more information.

Nominations are being accepted for the 2010 PB Poorman Award. The UW System wishes to publicly recognize the distinguished and consistent achievements in service of LGBTQ people within the UW-System. The 2010 PB Poorman Award will be presented at the 5th Annual UW System LGBTQ Spring Conference and the 34th Annual Conference of the UW System Women’s Studies Consortium April 16-17, 2010, at UW-Whitewater.

Nomination materials should include:
-A letter of nomination addressing one or more of the criteria listed above. (Nominations may be made by one’s self or others).
-A one- or two-page statement by the nominee on his or her professional contributions and service regarding LGBTQ people or issues.
-Three to four letters of support from colleagues or students qualified to comment on the nominee’s work. Where possible, the applicable nomination criteria from above should be addressed.
-A brief resume, not to exceed five pages, which documents the nominee’s performance.

SOCIAL SECURITY ADMINISTRATION ON CAMPUS IN NOVEMBER
A representative from the Janesville Social Security office will be on campus on Thursday, November 19 at 9 am in the UC, Room 259. The presentation will highlight retirement planning and social security benefits. There will be time for questions after the presentation.

The Office of Human Resources and Diversity must be notified within 30 days of changes in family status to be eligible for benefits. This includes marriage, divorce, birth, adoption, and death of any insured members.
